You're fairly limited with what you can do with the table of contents. It's
generated by the C++ code at runtime and isn't very flexible.

There are a couple of things you can do that may or may not be useful:

1. Adding the following to your collect.cfg will prevent the table of
contents from being displayed at all:
format DocumentContents false

2. You can use format statements to control each line of your table of
contents, just as you use them to control classification listings. For
example, try adding the following to your collect.cfg file and see what
format DocumentVList "some random stuff"
You should be able to use this to implement your third point above. With
clever use of metadata and format statements you might be able to do the
other two as well.
