Dr Fred Sai has devoted his entire career to issues of health in its most comprehensive form, He is best known for drawing attention to the food and nutrition problems of Africa, particularly in connection with children, and then as an advocate for women's rights and health. He has also devoted much attention to family planning, both as a human right and as a health measure, and was one of the founding members of the Planned Parenthood Association of Ghana. When the University of Ghana invited him to deliver the Aggrey-Fraser-Guggisberg Memorial Lectures in March 1994, he chose to speak about African women, and how their lives and health can be improved to the benefit of African families and all society.
