Good question. I've just had a quick try at this, and you are right, it
doesn't look like DocumentText is used for the header page. Not sure why
this is. You can get around this by modifying the DocumentHeading format
and putting stuff that you want in the header page in the heading
instead. [NumPages] is a good metadata to use for the test of header
page or not - the header page will have NumPages metadata while the
other pages wont. The problem with this is that the table of contents
(which may be a goto page box) will appear under heading. If you dont
like this, try using the extended formatting options, and then you get a
lot more control over the heading format.
Using the extended options disables the old DocumentContents,
DocumentButtons and DocumentImages format statements, so these things
need to be specified by hand.
The entire document page is now specified by DocumentHeading and
Here is an example:

format AllowExtendedOptions true
format DocumentHeading "<center><table width=537><tr><td>
{If}{[NumPages], [DocTOC]<p>[metadata or stuff that you want for the
heading page], [Title][DocTOC]}</td></tr></table>"

format DocumentText ".. use what you had previously ..."

Have a play with this and see if you can get what you want.
