Yes it does. For example, the file contains the core translations for a language, and any extra translations (generally) are put in the file.

  But if someone out there happens to have done that work for him or herself, would it be possible to get hold of it?  If I wanted, say, just a home page for Portuguese, I guess I could create a file with just the couple of bits in it I wanted?
Yes you can, remember to add the new macro file into the main.cfg file so it will be included.
