Preface and acknowledgements

Our thanks go to a number of people who have valuably contributed to the production of these Costing Guidelines. Charlotte Watts, Patrick Vaughan and Anthony Zwi, from the Health Policy Unit, LSHTM, have provided invaluable support and feedback since the inception of this project.
