Futher to what Michael said, if the mode attribute is omitted from a
Metadata element then it is taken to be override by default. So the
Librarian only writes in the mode attribute when it has been told to
accumulate some metadata and there could be confusion between what's
accumulated and what's overridden (especially when dealing with inherited
metadata!)
